Bill for Remote Therapeutic Monitoring done at home

Last updated: Nov 4, 2026 2 min read

Remote Therapeutic Monitoring (RTM) in Planetrehab records therapy the patient completes at home and gives the clinic a way to bill for that work. The activity log is the source for the claim, so it needs to be reviewed before anyone sends it.

Enroll the patient

RTM starts with a patient who has agreed to record home therapy and a staff member who will watch the period. Enroll the patient from the chart, confirm the monitoring dates, and show them how to record a session. If they cannot sign in to the portal, fix that before the period starts or the log will be empty when billing looks for it.

Tell the patient which home program counts. A walk they forgot to record, or a session logged on the wrong day, will not support the time you meant to bill.

Review the monitoring period

  1. Open the RTM activity for the patient and the monitoring period.
  2. Read the recorded sessions, including the dates and the minutes.
  3. Confirm the recorded home therapy meets the time you intend to bill.
  4. Note any gap, such as a week with no sessions, before you create a charge.

Check the activity log before billing so the claim matches what the patient actually completed. Do not round the log up to a threshold the record does not show.

Create the claim

When the period qualifies, create the RTM claim from the billing screen and submit it with the rest of the clinic’s claims. The charge should point at the same patient, dates, and minutes you just reviewed. Billing can see the claim status next to in-clinic visits on the worklist.

  • Use the codes and minutes your payer expects for RTM.
  • Keep the supporting log available if the claim is questioned.
  • Do not submit a second claim for the same monitoring period.

If the log is short

If the patient did not record enough home therapy, do not force the claim. Call or message them, confirm they know how to log a session, and let the next period start clean. A denied RTM claim is harder to fix than a period you chose not to bill.

Staff should look at open periods before the month closes so a qualifying log is not left unbilled, and a short log is not submitted by habit.
